After completing the registration form, You will need to log into your OFA account at Online.OFA.org and create new applications for all clearances requested. For CAER eye exams select "in a veterinary office". Then search for Amy Hunkeler, DVM, DACVO. For other exams see the name of the DVM providing the service.

Please remenber to bring your forms with you to the clinic. Failure to do so will result in you having to fill them out at the clinic and you will not be placed in line until all forms are completed.

Prices DO NOT include OFA fees. You will have to pay for these separately when you send in your completed forms.
