Dance Classes Registration Form

Terms and Conditions:

By submitting this form, I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Elevate Movement and Dance classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Elevate Movement and Dance, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Elevate Movement and Dance. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Elevate Movement and Dance, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, or injury. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. I have read, understood and agree to be bound by the above statement.

ELEVATE MOVEMENT AND DANCE POLICIES

Communications

1. I understand that Elevate Movement and Dance will communicate with me throughout the year via email, take home flyers and text messages. I understand it is my responsibility to read them with my child so that I am aware of billing cycles, dance recitals, or outside performances, enrollment for the following cycle, any attire or footwear my child may need; or any other important information that being a parent at Elevate Movement and Dance requires me to be informed of. I understand that the Elevate Movement and Dance schedule is subject to change.

Billing Obligations 

2. I understand that the Elevate Movement and Dance tuition is NON REFUNDABLE. NO CREDIT or REFUND is given for missed classes. Cash, Check and Credit Card are accepted. 

3. I understand that I will be charged monthly for classes. Monthly tuition is due on the first Friday of each month. A $5.00 late fee will be charged for fees that are past due. 

4. I understand that invoices will be sent through email.

5. I understand that families (same household) with more than one student may receive a family discount. 

1 student -$35/month per student 

Family Discount -$30/ month per student

 

6. I understand that it is my responsibility to purchase the necessary dance attire for classes and performances. Students will need white tennis shoes, black leggings (sweats for boys), a team shirt and also clogging shoes. Clogging shoes can be rented from our studio and must be returned in good condition. Rentals fees will be invoiced and sent to your personal email. 

7. I understand there is a $35.00 costume fee for all recitals.I understand that all fees must be paid in full before costumes are distributed. 

 

Attendance Policy 

8. I understand that Dress Rehearsal Practices, Recitals and Competitions are mandatory. 

9. I understand that Elevate Movement and Dance does allow three absences per season. If more than three practices are missed the student will not be allowed to perform in the season recital or competitions. It is the responsibility of each student to learn what is taught in class. If a student’s performance is not proficient they may not participate in performances or recitals. 

10. I understand it is my responsibility to be on-time. I understand that I need to pick my child up after class. I understand that I must not leave my child at the studio longer than THIRTY MINUTES after class has been dismissed. After THIRTY MINUTES you will receive a phone call to pick up your child. 

Class Dress Code 

11. I understand that my child’s hair MUST be up, or out of their face to participate in class. 

12. I understand that my child must wear appropriate dance attire to class. 

You'll need an outfit that is comfortable enough to move freely in.
Required dance shoes. 

 

Conduct

13. Students and parents will conduct themselves in an appropriate manner while at Elevate Movement and Dance  practices and at all performances and events in which Elevate Movement and Dance are being represented. Proper respect for instructors is expected of all students, parents/guardians and siblings. 

 

 Responsibilities of Property

14. I understand that Elevate Movement and Dance is not responsible for lost or stolen valuables. If your item has been misplaced please check our Lost and Found. Items must be claimed in person. Every 30-days, unclaimed items are given to a local charity. 

15. I understand if clogging shoes are lost, it is the responsibility of the student and parents to replace rented clogging shoes. 

 

Drop Off and Pickup

16. I understand that Elevate Movement and Dance is not responsible for injuries or accidents in the gym or parking lot. It is my responsibility to be careful backing out, and pulling in the lot; and exercise caution as children are always walking to-and-from class.

Clogging Shoe Policy 

17. All students must have clogging shoes that fit and are in good condition. Clogging shoes can be ordered through the Elevate Movement and Dance at full cost. Shoes can also be rented for $25.00. Once the season is over, the rented shoes must be returned to Elevate Movement and Dance. If the shoes are not returned the student will be charged the full price of the shoe. If the shoes are purchased they are the property of the student.
