Krusty & Stokes K9 Rescue Adoption Application
Krusty & Stokes K9 Rescue is an at home foster based small dog animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals. Krusty and Stokes K9 Rescue is completely funded through the founders of the organization. All of our pets are cared for by foster homes and receive complete medical care, which includes deworming, vaccinations, spaying/neutering and microchipping prior to their adoption. All adoptees will receive an invitation to a Free educational workshop. Dog training resources are also available for adopted dogs at a discount.
This questionnaire is designed to help Krusty & Stokes K9 Rescue and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. Due to us being a smaller based rescue we do not offer out of state adoptions and we only adopt out to the following counties in Georgia: Cobb, Douglas & Paulding. Krusty & Stokes K9 Rescue reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!
